Services

Coming home from the hospital, planned and supported.

The days after a hospital or rehab stay are when small things slip: a missed follow-up, a medication change no one explained, a home that is not ready. We help your family plan the move home and get the first weeks set up well.

When this helps

Is this where your family is right now?

Families usually reach out for hospital-to-home support in moments like these.

  • Your parent is moving from rehab back home
  • New medications or instructions feel confusing
  • The home needs equipment or safety changes

Good to know

  • Timing matters. Call as soon as you know discharge is coming, even if the date is not set.

Questions families ask

Frequently asked questions

When should we call?

As soon as you hear the word discharge. A few days of notice lets us help shape the plan instead of reacting to it. If your parent is already home and things feel shaky, call us anyway.

Can you help if I live out of state?

Yes. Many of the families we help are long distance. We can be your local presence during the transition and keep you updated by phone, text or email.

Is this covered by Medicare?

No. Care management is private pay. Some long-term care insurance policies reimburse part of the cost, and we can provide documentation for your claim.

Do you provide hands-on care?

No. We plan, coordinate and oversee. If your parent needs home health, aides or therapy, we help you choose and set those up, then keep an eye on how it is going.
